What to Bring to Your MedSurg Visit

Bringing the right items to your visit helps our clinical team see you faster and complete any required paperwork accurately.
Please bring
- A valid photo ID (driver's license, state ID, or passport)
- Your insurance card, if you have one
- A list of current medications, including doses
- Any relevant medical history or allergy information
- For physicals: the form provided by your school, employer, or organization
- For workers' compensation: your claim number and employer contact information
For minors
A parent or legal guardian must accompany patients under 18. Please bring guardianship paperwork if another adult is bringing the child.
If you do not have insurance, ask us about self-pay pricing and the MedSurg Access Membership, which can lower the cost of routine visits.
